New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.

You will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both of these forms are available as downloads from the above "Registering Online" tab.

All new patients over the age of five are offered a “New Patient Check up” with the practice nurse. This is strongly recommended as it ensures we have some knowledge of your past and present history. Appointments may be made on registering or by telephoning 01725 510296.
Medical treatment is available from the date of registration. Please contact reception for further information.
Communication Methods
How can we communicate with you? We can phone, video call, email, text or use other means if you are visually impaired or have hearing difficulties. Bring this form in to reception with you, or let reception know how we can help.
